Abstract Prevalence of premenstrual syndrome in female students in Surabaya is: 39.2% experienced severe symptoms and 60.8% experienced mild symptoms. 70% of students at Midwifery Department of Bangkalan, Health Polytechnic of Surabaya, experience premenstrual syndrome symptoms. The aim of the study was to determine the correlation of physical activity and nutritional status to the incidence of premenstrual syndrome, using cross-sectional design. The population were students of Midwifery Department of Bangkalan, Health Polytechnic of Surabaya. The sample size were 119 students, selected by simple random sampling. Data were collected using questionnaires, stature meters and scales, then analyzed Sperman rank test. Statistical tests showed a positive correlation between physical activity and incidence of PMS and positive correlation between nutritional status and incidence PMS.
