Surgical management of oro-antral fistula post-medication related osteonecrosis of maxilla: A case report

Abstract

With emerging cases of COVID-19 pandemic throughout the world, affecting more than 120 million people and causing more than 2 million worldwide deaths, definitive treatment is yet to be established. Meanwhile, the treatment protocol is largely based on resolving the cytokine storm and lung injury using high dose steroids, broad-spectrum antibiotics and invasive and non-invasive ventilation as a life saving measure. The current case report describes a case of delayed presentation of osteonecrosis of maxilla in an immune-compromised patient with co-morbid conditions after COVID-19 treatment, leading to loss of maxillary alveolar bone. Osteonecrosis of maxilla is a rare occurrence considering the perfusion of the bone. In the current case it was noted right after the patient recovered from COVID-19 infection, before which the patient was asymptomatic. Medical history revealed judicious use of antibiotics and corticosteroids to control the acute respiratory distress caused by COVID-19 followed by the osteonecrosis of alveolar bone in maxilla suggesting medication related osteonecrosis. The osteonecrosis went undetected due to delay in reporting of the case and thus severely affecting patient's Quality of Life. The procedure for surgical correction of the defect from loss of maxillary structure has been furt her discussed.
