Online published on 25 November, 2013. Abstract As more and more posterior teeth are being saved endodontically, the complications have become a matter of concern to the endodontists. Paraesthesia has been reported as a major complication during surgical and non-surgical endodontic therapy in mandibular posterior teeth. This is due to injury to the contents of the mandibular canal which has a close anatomic relationship with the root apices of the mandibular posterior teeth. Injury may be due to the neurotoxic effects of the root canal cements or over instrumentation or over filling. This unpleasant and painful complication can be minimised if due attention is given to the anatomical relationship of the mandibular canal to its surroundings. For this pupose the present study was carried out to know the exact anatomic distances of the mandibular canal from the root apices of the mandibular second premolar to second molar in vertical as well as horizontal direction. Twenty five human cadaver mandibles were cut in the midline to make the specimen number as fifty. In each specimen a total number of five sections were cut along the roots of second molar to second premolar teeth.
