Abstract Orthokeratinized odontogenic keratocyst cyst (OOC) is a developmental cyst of jaw, initially considered by the World Health Organization (1992) as an uncommon variant of Keratocyst odontogenic tumor (KCOT). However, OOC presents peculiar clinicopathologic features which is at variance with other developmental odontogenic cysts, especially OKC. The purpose of the article is to discuss a rare OOC arising in the anterior maxilla and to review the pertinent literature.
