Maxillary to mandibular tooth width ratio, as proposed by Wayne Bolton (1958), is an important diagnostic tool for predicting the results of orthodontic treatment. Bennett and McLaughlin, (1993) added ‘correct tooth size’ as the seventh key. It is important that the maxillary and mandibular teeth must be proportional in size in order to achieve the first six keys. A proper relationship of the total mesiodistal width of the maxillary dentition to that of the mandibular dentition favors an optimal post treatment occlusion and stability. Aim To assess the reliability of Bolton's analysis as a viable diagnostic tool in different angle's malocclusions. Methodology Study models of 60 patients aging from 18-30years were selected from OPD. This was divided into 20 each of skeletal and dental Angle's Class I, II and III. The mesiodistal tooth width from permanent canine to canine was measured on the study models with the help of a digital vernier caliper and Bolton's Anterior Ratio (AR) was calculated. Results 1) Individuals with Class III malocclusion have significantly greater AR than other groups. 2) The mean AR for Class III was significantly higher as compared to Class I. Conclusion This Bolton discrepancy can be attributed to the increased width of mandibular incisors and may be due to etiological factors that lead to mandibular prognathism.
