A Cross Sectional Study to Assess the Agreement between Stevenson & Tanner-Whitehouse Method for Bone Age Estimation

Abstract

Bone age is an indicator of the skeletal and biological maturity of an individual. This is different from chronological age. The TW2 method relies on the systematic evaluation of the maturity of all the bones in the hand and wrist and gives a more accurate estimation of age and reproducible, it is less frequently used as it is more complex and requires more time. Comparison of bone age estimated by Tanner Whitehouse (TW2 method) with Stevenson method in Indian population was undertaken in this study. 53 digital x-rays of left wrist joint antero-posterior view of males taken during 2016 to 2018 were included. Blinded expert estimated bone age using TW2 method and Stevenson method. Unblinded expert analyzed the demographic, physical and date of birth and then compared the agreement between age estimation calculated by the blinded forensic expert with the age calculation from date of birth with Bland Altman analysis. The mean bone age as estimated by TW2 method was 14 ±1.6 yrs. The mean lower limit (LL) of bone age estimated by Stevenson method was 14.2 ± 2.3yrs while the mean upper limit (UL) of bone age estimated was 16.2 ± 1.76 yrs. This study demonstrated that the bone age estimated by Tanner-Whitehouse method is in close agreement with birth age (chronological age) and also in acceptable agreement with LL of bone age estimated by Stevenson method.
