Abstract Background The Health Information System (HIS) condition in Indonesia is not at an optimum level yet, and there is still much to be done to improve and strengthen all HIS components. The objectives of this study were to evaluate whether the district-focused approach could improve the performance of district-level HIS networks in Indonesia. Method The study was carried out to evaluate the project, which was implemented in ten districts in five provinces of Indonesia. This evaluation study used two approaches: a) the Health Metric Network (HMN) framework from the World Health Organization (WHO), and b) the System Success Model from DeLone and McLean. Results Initial assessment results in early 2017 were compared to those at the end of the project in May 2018, and HIS performance increased for each component in all districts. However, the achievements were not fully optimized, and there is room for further strengthening. Most participants of the System Success Model assessment reported that HIS strengthening was successful and useful, with 89% improvement in system quality, 85% in information quality, 93% in service quality, 96% in individual impact, and 89% in organizational impact.
