Online published on 13 November, 2019. Abstract All teeth are found to have variations in dental anatomy. For the success of endodontic treatment, clinician should have complete knowledge regarding the variations of the root canal anatomy. This case report demonstrates successful management of maxillary first molar with six canals: two palatal, two mesio-buccal and two disto-buccal canals. Such variation in anatomy exists and so the astute clinician should take a note of these anatomical considerations while treating a tooth.
