The Efficacy of Prophylactic Imepemen in Preventing Post-Ureterorenoscopy Fever

Abstract

Background

Ureteroscopic removal of stones (URS) has been widely used to treat ureteral stones because it is comparatively safe, has a high success rate, and enables patients to rapidly return to their daily routines 1. However, some patients experience fever after URS, but the incidence of post-URS fever(18.3%)1. This study aimed to investigate the the efficacy of imipenemprophylaxis in preventing post-ureterorenoscope(URS) fever.

Imipenemmedication antibiotics used to treat a wide variety of bacterial infections. This medication is known as a carbapenem-type antibiotic. It works by stopping the growth of bacteria Imipenem is a broad-spectrum betalactam antibiotic used for severe bacterial infections caused by susceptible organisms. It has activity against many aerobic and anaerobic Gram-positive and Gram-negative organisms, including Staphylococcus aureus, Streptococcus pyogenes, S. agalactiae, S. viridans-group streptococci, Enterococcus faecalis, Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis, Bacteroidesfragilis and Peptostreptococcus species.

In this study47 patients(32 male and 15 females) were subjected to URS for stone in the ureter, all were given prophylactic dose of imipenem infusion 250mg, at time of induction of anesthesia to study the efficacy of the drug in preventing post-URS fever.only 3 patients(1 male and 2 females) where there stones at the upper third of the ureter suffered from fever in day zero postoperatively with incidence of (6.4%), treated by additional night dose of the same drug 500mg and paracetamol1000mg intravenous infusion. We concluded that prophylactic imipenem dose of 250 mg intravenous infusion is associated with a signifcant decrease of post-URS fever.
