Online published on 6 March, 2013. Abstract Carcinoma ex pleomorphic adenoma is a rare malignant neoplasm of the salivary glands. It is a subtype of malignant mixed tumor that has its subdivisions, such as Carcinoma Ex-pleomorphic adenoma (CEPA), Carcinosarcoma and metastizing pleomorphic adenoma. All subtypes must necessarily be composed of some quantity of pleomorphic adenoma, as their transformation into CEPA occurs in 1% to 7% of cases. The female gender is a little more affected and there is greater incidence in middle-aged patients with prevalence from the 5th to 7th decade of life. In over 80% of cases, CEPA occurs in the major salivary glands and the most affected gland is the parotid, however, it may also occur in the minor salivary glands. Patients may report rapid growth with pain and ulceration, whereas they may usually present as painless. The aim of the present study is to report a case of an carcinoma ex pleomorphic ex adenoma of large proportions in the right palatine region.
