Oral Pulse Granuloma: A Case Report

Abstract

Oral Pulse granuloma results from the implantation of food particles of plant or vegetable origin. In this case the vegetable matters are introduced through an alveolar defect caused by post -extraction socket. Pulse granuloma is also called as vegetable granuloma. Based on histopathological features it is also called as Hyaline ring granuloma. Lesion is benign in nature, considered to be foreign body reaction to vegetable particles. This article represents a case with patient complaining of non-healing wound with pain and swelling after a tooth extraction. After a thorough investigation and examination a wide ulcero-proliferative growth was seen in the alveolus of lower left first molar extending posteriorly to the neighboring edentulous area and even to the buccal mucosa measuring 3x3 cm with a raised and indurated border mimicking squamous cell carcinoma. An incision biopsy was done under local anesthesia. The histological picture gave the impression of oral pulse granuloma revealing hyaline rings surrounded by inflamed granulation tissue with associated multinuclear giant cells. The wound was minimally debrided, irrigated with saline and povidone iodine. Oral antibiotics were administered for 5 days and reviewed later. The wound was completely healed leaving a small nodule which was excised.
