Abstract This study is an attempt to analyse the existing health care delivery in India to identify the issues and challenges existing in the current health care scenario. To conduct the study secondary data has been collected from different published reports and governmental records. The study focuses on health infrastructure considering the primary health centre, sub centre and community health centres existing in year 2005 and year 2015 and examine the shortage of health care infrastructure in major states of India. India's total healthcare expenditure as a per cent of GDP is still one of the lowest in world, 1.4 percent of GDP compared to global average 10.4 percent and public sector accounts for only around 30 percent of total healthcare expenditure (World Bank Report 2014). In order to improve public health infrastructure, public health expenditure is essential. The study recommends improved government spending on health care delivery by strengthening health care infrastructure, like number of PHC's, SC's, CHC's and doctors, beds are less than the requirement and to fill this inadequacy, public health expenditure is required. When number of patients depending on public health centres is more it becomes difficult to provide with quality health care. Therefore, public expenditure plays an important role in creating health infrastructure, providing stable and reliable health facilities to people.
