Abstract Submucosal vital root retention is one of the valid means of retaining an adequate residual bony alveolar ridge to a greater degree than when patients are rendered totally edentulous. In sub mucosal vital root retention procedure the vital tooth root portion, till the alveolar crest level is retained under the full thickness flap, after surgical removal of the crown portion, to prevent rapid resorption of the alveolar ridge. Alveolar bone maintenance depends on the presence of healthy root and periodontal ligament, which transmits functional and parafunctional forces to surrounding bone. Various techniques including root retention and purposeful implants are available for alveolar ridge preservation. Studies have shown that submucosal vital root retention also maintains a clinically acceptable physiology of residual ridges. An approach to soft tissue management and surgical crown/root sectioning for submucosal vital root retention has been presented.
