A Comparative Evaluation Of Anesthetic Efficacy Between Articaine, Lidocaine, Bupivacaine And Mepivacaine In Patients With Irreversible Pulpitis Of Mandibular Molar: -A Randomized Control Trial.

Abstract

Aim

To compare the anesthetic efficacy of four different types of local anesthetic agents in patients with irreversible pulpitis of mandibular molar.

Material and Methods

The patients (200) diagnosed with irreversible pulpitis and exhibiting positive response to electric pulp test and prolonged response to cold testing with Endo frost in the mandibular posterior tooth were selected. These patients assigned for RCT and received a IAN Block to anesthetize the tooth. During the pulpectomy procedure, the patients were instructed to report any painful discomfort. To evaluate the intensity of pain during the pulpectomy, a Verbal Analogue Scale (0–3) was used.

Results

The results of the present study demonstrated 66% success rate for Articaine followed by Lidocaine (36%) and Mepivacaine (36%). Bupivacaine showed least efficacy (22%).

Conclusion

For mandibular posterior teeth with irreversible pulpitis, neither of the anesthetic solution, administered in a conventional IAN block, resulted in an acceptable rate of anesthetic success. There was significantly less pain with Articaine group as compared to other three groups
